We will reach out to schedule and begin the pre-appointment intake process. This will include providing you with access to our online patient portal where you can fill out some basic information and provide us consent to treat. Your appointment will take place within that same portal utilizing HIPAA-compliant video-conferencing software. During the intake, we may ask additional information.

Please do your best to gather the following information as you prepare for your intake:

  1. Any previous genetic testing results.

  2. If you have had any major biopsies or surgeries, a copy of the clinical or pathology report.

  3. Your biological family’s health history (any grandparents, parents, aunts/uncles, first-cousins, siblings, or children)

    • If any relatives have had genetic testing, any information about those results or a copy may be useful. Important details include: the year of the test, lab name, which genes were tested, and as much detail is available about the results.

  4. A copy of your insurance card and/or payment method.
